Curative effect of ivabradine and its influence on soluble stromelysin-2 in elderly patients with chronic heart failure

Abstract:Objective To investigate the curative effect of ivabradine and its influence on soluble stromelysin-2(sST2) in elderly patients with chronic heart failure(CHF).Methods A total of 52 patients with CHF in Fuwai Hospital,Chinese Academy of Medical Sciences and Beijing Anzhen Hospital,Capital Medical University from August 2015 to February 2016 were enrolled.All patients were treated with standard CHF therapy and ivabradine for 3 months,the initial dose of ivabradine was 2.5 mg/time,2 times/d,the dosage was adjusted to control the heart rate around 60/min,the maximum dose was 7.5 mg/time,2 times/d.Heart rate,left ventricular ejection fraction(LVEF),N-terminal pro-brain natriuretic peptide(NT-proBNP) and sST2 levels before and after treatment were recorded.Correlations among heart rate,NT-proBNP and sST2 were analyzed.Adverse reactions were observed.Results After 3 months of treatment,heart rate,NT-proBNP and sST2 levels were decreased and LVEF was increased[(70 ± 10)times/min vs(80 ±8)times/min,1 682(212,3 628)ng/L vs 2 450(254,5 344)ng/L,57 (38,144) μg/L vs 78 (43,185) μg/L,(40 ± 6) % vs (31 ± 7) %],the differences were statistically significant(P < 0.05).The absolute value of heart rate decrease [(10 ± 9)times/min] was correlated with the median absolute value of NT-proBNP decrease(756 ng/L) (r =0.682,P =0.006) and the median absolute value of sST2 decrease(19 μg/L) (r =0.691,P =0.001).Few adverse effects were observed.Conclusion Ivabradine is safe and effective in the treatment of CHF in elderly patients,it can significantly decrease the levels of blood biomarkers.
