Clinical study of different chemotherapies combined with stereotactic body radiation therapy treating advanced pancreatic cancer in elderly patients

Abstract:Objective To analyze effectiveness and safety of CyberKnife stereotactic body radiation therapy (SBRT) combined with gemcitabine or tegafur/gimeracil/oteracil potassium(TS-1) chemotherapy in the treatment of advanced pancreatic cancer in elderly patients.Methods Totally 60 patients aged 65-79 years with locally advanced or metastatic pancreatic cancer from August 2013 to August 2015 in Navy General Hospital were enrolled.All patients bad CyberKnife SBRT(5-8 Gy/f,25-40 Gy/5f,5 f/week) due to primary tumor and metastatic retroperitoneal lymph nodes and were divided into 2 groups by different chemotherapy plans:G group(gemcitabine,1 g/time,1 time/d,d1,d8) and S group(TS-1,40 mg/time,2 times/d,d1-14),with 30 cases in each group.Senquential chemotherapies were carried out for at least 4 cycles 3 weeks after radiotherapy.Effective rate,adverse reactions,overall survival(OS) and progression free survival(PFS) were analyzed through follow-up.Results Three months after radiotherapy,effective rates in G group and S group were 80.0% (24/30) and 76.7% (23/30) respectively;after sequential chemotherapy,effective rates in G group and S group were 63.3% (19/30) and 60.0% (18/30) respectively;differences of effective rates were not significant between groups (P =0.754,P =0.791).The incidence of adverse reactions had no significant difference between groups (P > 0.05).Median OS in G group and S group were 10.0 and 11.5 months;OS rates of 0.5 year,1 year,2 years were 93.3% (28/30),40.0% (12/30),10.0% (3/30) and 86.7% (26/30),46.7% (14/30),13.3% (4/30),respectively;median PFS were 10.0 and 9.0 months;PFS rates of 0.5 year,1 year,2 years were 76.7% (23/30),20.0% (6/30),3.3% (1/30) and 80.0% (24/30),23.3% (7/30),3.3% (1/30),respectively;differences of OS and PFS were not significant between groups (P =0.977,P =0.946).Conclusion CyberKnife SBRT combined with gemcitabine or TS-1 chemotherapy is safe and effective for locally advanced and metastatic pancreatic cancer in elderly patients.
