Clinical effect of early enteral and parenteral nutritional support for patients with advanced esophageal cancer during chemotherapy

Abstract:Objective To investigate the clinical effect of combined enteral and parenteral nutrition support in patients with advanced esophageal cancer during chemotherapy.Methods Totally 76 patients with advanced esophageal cancer from August 2012 to November 2015 in People's Hospital Affiliated to Hubei University of Medicine were randomly divided into observation group and control group,with 38 cases in each group.During chemotherapy,the control group had parenteral nutrition support;the observation group had combined enteral and parenteral nutrition support;both treatments lasted for 3 months.Levels of total protein,albumin and immunoglobulin(Ig) A,IgM and the incidence of complications were analyzed.Results Before treatment,total protein and albumin had no significant differences between groups [(67.5 ± 2.4) g/L vs (68.1 ± 3.1) g/L,(39.1 ±2.1)g/L vs (39.1 ±2.4)g/L] (P >0.05).After treatment,total protein and albumin in control group [(61.3 ±3.5)g/L,(33.2 ±4.6) g/L] had no significant differences compared to those before treatment;total protein and albumin in observation group [(66.5 ± 2.4) g/L,(39.9 ± 4.1) g/L] were significantly lower than those before treatment and those in control group(P <0.05).After treatment,IgM and IgA levels were significantly higher than those before treatment in both groups[control group:(1.6 ±0.3)g/L vs (1.1 ±0.4)g/L,(2.8 ± 0.5) g/L vs (1.7 ±0.5)g/L;observation group:(1.9 ±0.4)g/L vs (1.2 ±0.4)g/L,(3.7 ±0.8)g/L vs (1.7 ± 0.6)g/L] (P < 0.05).IgM and IgA levels in observation group were significantly higher than those in control group(P < 0.05).Both groups had pulmonary infection,arrhythmia,gastrointestinal reactions and other complications during treatment,but the total incidence had no significant difference between groups (P > 0.05);all complications were alleviated after symptomatic treatments.Conclusion Combined enteral and parenteral nutritional support for patients with advanced esophageal cancer during chemotherapy is more beneficial for nutritional status and immune function recovery compared to parenteral nutrition support.
