Effect of dexmedetomidine and propofol on intraoperative awakening in cerebral function area operation

Abstract:Objective To observe effects of dexmedetomidine and propofol on intraoperative awakening during cerebral function area operation.Methods Totally 22 patients who had cerebral function area operation and required intraoperative awakening from September 2013 to December 2015 in Chenggong Hospital Affiliated to Xiamen University were divided into dexmedetomidine group and propofol group,with 11 cases in each group.Propofol and remifentanil were used for anesthesia maintenance.For dexmedetomidine group,propofol infusion was blocked 15 min before electrocorticogram monitoring;dexmedetomidine was intravenous injected with a loading dose of 0.8 μg/kg(0.3 μg/kg for continuous intravenous infusion);before awakening,dexmedetomidine infusion rate was set at 0.1 μg/(kg · h),target effect-site concentration of remifentanil was set at 1.0-1.5 μg/L.Propofol group was continuously infused propofol;before awakening,target plasma concentration of propofol was regulated at 1.0-1.5 mg/L,target effect-site concentration of remifentanil was set at 1.0-1.5 mg/L.Heart rate,mean arterial pressure(MAP) and blood gas indexes[arterial partial pressure of oxygen(PaO2) and arterial partial pressure of carbon dioxide (PaCO2)] were recorded at 5 time points:before operation (T0),upon endotracheal intubation (T1),25 min after opening the skull(T2),15 min after awakening(T3),15 min after the second time of anesthesia(T4).Serum glucose,serum interleukin-6(IL-6) and plasma cortisol were tested.Occurrences of agitation and bucking,usage of vasoactive drugs,intraoperative awareness rate and other adverse events were recorded.Results In dexmedetomidine group,heart rates at T2 and T3 were significantly lower than those at T0 and T1 [(65 ±9),(62 ± 11) times/min vs (80 ± 14),(78 ±9) times/min] (P <0.05).There was no significant difference of MAP between groups at each time point(P > 0.05).At T3,PaO2 and PaCO2 had no significant differences between groups (P > 0.05).Serum glucose had no significant differences between groups and among T0-T4 points (P > 0.05).In propofol group,IL-6 was constantly increased from T0 to T4,but the difference between groups was not significant(P > 0.05).The difference of cortisol between groups was not significant (P > 0.05).During awakening period,incidences of agitation and bucking,usage ratio of vasoactive drugs and intraoperative awareness rate had no significant differences between groups (P > 0.05).No other adverse events occurred in both groups.Conclusion Dexmedetomidine and propofol have same sedative effect on intraoperative awakening during cerebral function area operation.
