Effect and safety of dexamethasone in treating gastric acid aspiration induced acute lung injury

Abstract:Objective To explore the effect and safety of dexamethasone in treating gastric acid aspiration induced acute lung injury (ALI).Methods Totally 54 patients with gastric acid aspiration induced acute lung injury (ALI) were randomly divided into observation group and control group (27 cases in each group).Both groups were given treatment of primary disease,mechanical ventilation and infusion;observation group was additional given dexamethasone (0.75-2.25 mg/time,2-4 times/d,oral;6-18 mg/times,intravenous drip,repeated administration per 4-6 h till disease was stable).The acute physiology and chronic bealth evaluation Ⅱ (APACHE Ⅱ)score,positive end expiratory pressure (PEEP),oxygenation index,duration of mechanical ventilation,intensive care unit (ICU) stay time and adverse drug reactions before and 1,3,5 d after treatment were compared between groups.Results The APACHE Ⅱ score was not significantly different between groups at each time point (P >0.05).In observation group,the oxygenation index was significantly lower than that in control group before treatment,while was significantly higher than that in control group 5 d after treatment [(191 ±25) mmHg vs (211 ±29) mmHg,(281 ±24) mmHg vs (246 ±32) mmHg] (P <0.05),and was not significantly different between groups 1 and 3 d after treatment (P > 0.05).The PEEP was not significantly different between groups before treatment (P>0.05);1,3,5 d after treatment,it was significantly lower in observation group than that in control group [(4.5 ± 1.5) cmH2 O vs (7.1 ± 1.5) cmH2 O,(6.2 ± 2.8) cmH2 O vs (8.5 ± 2.1) cmH2 O,(4.3 ±1.0) cmH2 O vs (8.4 ± 2.3) cmH2 O] (P < 0.05).The duration of mechanical ventilation,ICU stay time in ob-servation group were significantly shorter than those in control group [(5.5 ± 1.9) d vs (9.3 ±2.2) d,(10.2 ±4.3) d vs (16.7 ± 5.5) d] (P < O.01).No severe adverse reactions occurred in both groups.Conclusion On the basis of conventional treatment,dexamethasone can effectively and safely improve hypoxemia and lung compliance in treating gastric acid aspiration induced ALL
