Application of ticagrelor in emergency percutaneous coronary intervention in patients with acute myocardial infarction

Abstract:Objective To explore the application of ticagrelor in emergency percutaneous coronary intervention (PCI) in patients with acute myocardial infarction.Methods Totally 188 patients with acute myocardial infarction who underwent emergency PCI from June 2014 to May 2015 were divided into clopidogrel group and ticagrelor group (94 cases in each group).Clopidogrel group was orally given loading dose of clopidogrel 600 mg and aspirin 300 mg before PCI,clopidogrel 75 mg(1 time/d) and aspirin 100 mg (1 time/d) after returning to the ward;ticagrelor group was orally given loading dose of ticagrelor 180 mg and aspirin 300 mg before PCI,ticagrelor 90 mg (2 times/d) and aspirin 100 mg (1 time/d) after returnning to the ward.The inhibition rate of adenosine phosphate (ADP) and maximum of the thrombus (MA) 0.5,1,2,4,6,12,24,48,72 h after taking medicine were measured by thrombelastogram and compared between groups.Results The inhibition rate of ADP was significantly higher,the MA was significantly lower in ticagrelor group than those in clopidogrel group 0.5,1,2,4 h ffter taking medicine [ADP inhibition rate:(41.1 ±10.1)% vs (8.3 ±1.2)%,(70.2±20.1)% vs (21.0±4.1)%,(88.6±19.9)% vs (38.4±10.2)%,(86.1 ±24.4)% vs (51.7 ±9.8)%;MA:(60 ± 17)% vs (64 ±20)%,(45 ± 10)% vs (56± 18)%,(44 ± 15)% vs (57 ±20)%,(36 ± 10)% vs (53 ±15)%] (P<0.01 or P<0.05).Conclusion Ticagrelor has a certain advantage in terms of antiplatelet compared with clopidogrel in patients with acute myocardial infarction who underwent emergency PCI.
