Analysis of glycometabolism in different nationality patients with coronary heart disease and/or hypertention in Aletai city

Abstract:Objective To understand the status of glycometabolism in different nationality patients with coronary heart disease and/or hypertention in Aletai city.Methods The glycometabolism of 900 patients with coronary heart disease and/or hypertention in Aletai city,including 480 cases of Kazak and 420 cases of Han,were retrospectively analyzed.Results There were 540 cases (60.0%) with glycometabolism abnormality,including 130 cases (24.1%) of diabetes mellitus and 410 cases (45.6%) of pathoglycemia.In 410 pathoglycemia patients,200 cases were Kazak mainly with increased random blood glucose [80.0% (160/200)],being significantly different with 210 cases of Han [57.1% (120/210)] (x2 =24.70,P < 0.01).Oral glucose tolerance test (OGTT) was performed in 120 cases who had fasting blood glucose 6.1 mmol/L and random blood glucose > 7.8 mmol/L,the result of which showed that 100 cases (83.3%) had abnormal glucose tolerance (60 cases of Kazak and 40 cases of Han).In 100 cases of abnormal glucose tolerance,20 cases were newly diagnosed as diabetes mellitus (12 cases of Kazak and 8 cases of Han),30 cases had impaird fasting blood glucose (14 cases of Kazak and 16 cases of Han),35 cases had increased postprandial blood glucose (27 cases of Kazak and 8 cases of Han),and 15 cases had increased fasting and postprandial blood glucose (7 cases of Kazak and 8cases of Hant);the incidence of abnormal postprandial blood glucose in Kazak was significantly higher than that in Han [45.0% (27/60) vs 20.0% (8/40)] (x2 =6.44,P <0.05).The average age of patients with impaired glucose tolerance was (50.1 ± 1.2) years.In ≥60 years old patients,220 cases had glycometabolism abnormality,90 patients (40.9%) had been definited as diabetes mellitus on admission among them;52 patients were found having abnormal glucose tolerance after OGTT examination,among them,15 cases were newly diagnosed as diabetes,20 cases had impaired fasting blood glucose,10 cases had increased postprandial blood glucose and 7 cases had both increased fasting and increased postprandial blood glucose.In <60 years old patients,320 cases had glycometabolism abnormality,40 patients (12.5%) had been definited as diabetes mellitus on admission among them;48 patients were found having abnormal glucose tolerance after OGTT examination,among them,5 cases were newly diagnosed as diabetes,10 cases had impaired fasting blood glucose,25 cases had increased postprandial blood glucose and 8 cases had both increased fasting and increased postprandial blood glucose.Patients of ≥60years old mainly had impaired fasting blood glucose and patients of < 60 years old mainly had increased postprandial blood glucose,with significant difference between them [impaired fasting blood glucose:38.5% (20/52) vs 20.8% (10/48),increased postprandial blood glucose:19.2% (10/52) vs 52.1% (25/48)] (x2 =15.00,P< 0.05).In 540 patients with abnormal glycometabolism,100 cases (18.5%) had coronary heart disease,280cases (51.9%) had hypertension,160 cases (29.6%) had hypertension combined with coronary heart disease.Conclusion At Aletai area,patients with hypertension and/or coronary heart disease had high incidence of abnormal glycometabolism;missed diagnosis of diabetes and impaired glucose tolerance is common simply relying on fasting blood glucose;detection of abnormal glycometabolism should be strengthened in patients with hypertension and/or coronary heart disease for early diagnosis and intervention of diabetes and impaired glucose tolerance.
