Effect of dexmedetomidine combined with flurbiprofen on hymodynamic in elderly patients with thyroidectomy

Abstract:Objective To evaluate the effects of dexmedetomidine (DEX) combined with flurbiprofen on hemodynamics in elderly patients with thyroidectomy.Methods Totally 60 elderly patients undergoing subtotal thyroidectomy from January to December in 2014 were randomly divided into observation group and control group (30 cases in each).Both groups were given midazolam 0.02 mg/kg,etomidate 0.3 mg/kg,fentanyl 2 μg/kg and rocuronium 0.6 mg/kg for anesthesia induction.Then observation group was given continuous intravenous infusion of DEX 0.4 μg/(kg · h) and intravenous injection of flurbiprofen 50 mg 30 min before the end of operation;control group was given equal dosage of 0.9% sodium chloride injection.The operation duration,postoperative eye open time,extubation time were recorded;the systolic pressure,diastolic blood pressure and heart rate before anesthesia (T0),when stripping the thyroid gland (T1),immediately after extubation (T2),2,4,8 h after extubation (T3,T4,T5),the Ramsay sedation score and pain visual analogue scale (VAS) score at T2-T5 point were recorded and compared between groups.Results The operation duration,postoperative eye open time,extubation time were not significantly different between groups [(97 ± 10) min vs (97 ± 9) min,(8.2 ± 2.0) min vs(7.7 ±3.6) min,(10.3 ±2.0) min vs (9.1 ±2.3) min] (P>0.05).At T1-T5 point,the diastolic pressure,systolic pressure and heart rate in observation group were significantly higher than those in control group[diastolic pressure:(120 ± 16) mmHg vs (133 ± 14) mmHg,(117 ± 11) mmHg vs (137 ±12) mmHg,(117 ±10)mmHgvs (136±15) mmHg,(121 ± 12) mmHg vs (138 ± 13) mmHg,(123 ± 12) mmHg vs (136 ±12) mmHg;systolic pressure:(77 ±8) mmHg vs (91 ±10) mmHg,(78 ±9) mmHg vs (89 ± 10) mmHg,(76 ±8) mmHg vs (87 ±8) mmHg,(79 ±9) mmHg vs (86 ±8) mmHg,(78 ±9) mmHg vs (87 ±9) mmHg;heart rate:(72 ± 13) times/min vs (87 ± 15) times/min,(77 ± 11) times/min vs (89 ± 12) times/min,(76 ±10) times/min vs (87 ±11) times/min,(77 ± 10) times/min vs (87 ± 11) times/min,(77 ±± 8) times/min vs(86 ± 10) times/min] (all P < 0.05).At T2-T5 point,the Ramsay sedation score in observation group were significantly higher than that in control group [(2.94 ± 0.51) scores vs (1.76 ± 0.23) scores,(2.51 ±0.24) scores vs (1.27 ± 0.17) scores,(2.47 ± 0.31) scores vs (1.31 ± 0.12) scores,(2.32 ±0.46) scores vs (1.33 ± 0.20) scores],the VAS in observation group were significantly lower than that in control group [(1.2±0.3) scores vs (4.0 ±0.8) scores,(1.9 ±0.4) scores vs (5.6 ±0.7) scores,(2.1 ±0.5) scores vs (6.0 ± 0.5) scores,(2.2 ± 0.6) scores vs (5.5 ± 0.6) scores] (P < 0.05).Conclusion In subtotal thyroidectomy,DEX combined with flurbiprofen has good analgesic effect,do not affect the wake up time of anesthesia,and can maintain hemodynamic stability in elderly patients.
