Effects of different types of antidiabetic drugs on serum insulin-like growth factor-1 and insulin-like growth factor binding protein-3 in patients with type 2 diabetes melitus

Abstract：Objective To investigate the effects of different types of antidiabetic drugs on serum insulinlike growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) in patients with type 2 diabetes melitus.Methods Totally 120 patients with type 2 diabetes were divided into insulin group,insulin combined with metformin group,insulin secretagogues group and metformin combined with acarbose group (30 cases in each group) according to types of antidiabetic drugs.Enzyme-linked immunosorbent assay was used to detect the levels IGF-1 and IGFBP-3 in serum,and the ratio of IGF-1/IGFBP-3 was calculated.Results The IGF-1 level and ratio of IGF-1/IGFBP-3 in metformin combined with acarbose group were significantly lower than those in insulin group,insulin combined with metformin group,insulin secrctagogues group [(130 ± 9) μg/L vs (149±13),(148 ±14),(146 ±14) μg/L;(0.39 ±0.08) vs (0.49 ±0.11),(0.46±0.09),(0.45 ±0.10) μg/L] (P ＜0.05).The serum IGFBP-3 level was not significantly different among groups (P ＞ 0.05).IGF-1 was positively correlated with fasting insulin (Fins) (r =0.593,P ＜ 0.001),was negatively correlated with fasting plasma glucose (FPG) (r =-0.427,P ＜ 0.001),was not correlated with IGFBP-3,body mass index and Homeostalic model assessment-insulin resistance (r =0.032,-0.025,-0.111;P =0.763,0.817,0.298).Ratio of IGF-1/IGFBP-3 was positively correlated with IGF-1 (r =0.784,P =0.000).FPG and Fins were independent factors of IGF-1 (β =-0.603,P =0.000;β =0.268,P =0.001).IGF-1 was an independent factor of ratio of IGF-1/IGFBP-3 (β =0.776,P ＜0.001).Conclusion Metformin combined with acarbose can reduce the serum IGF-1 level and ratio of IGF-1/IGFBP-3,this result is possible to provide a theoretical basis for individual treatment of diabetes.
