Effect of milrinone in treating chronic pulmonary heart disease complicated with severe heart failure

Abstract：Objective To observe the therapeutic effect of milrinone in treating chronic pulmonary heart disease complicated with severe heart failure.Methods Totally 68 cases with chronic pulmonary heart disease complicated with severe heart failure were enrolled and randomly divided into observation group and control group (34 cases in each group).After admission,both groups were given basic therapy;in addition,observation group was intravenously given milrinone 0.5μg/(kg · min) continuously with micro pump for 7d.The curative effect,cardiac function index [left ventricular end-diastolic diameter (LVEDD),left ventricular ejection fraction (LVEF),stroke volume (SV),pulmonary artery pressure],levels of N terminal B-type brain natriuretic peptide (NT-proBNP) and adverse reactions were observed and compared between groups.Resutls The total effective rate in observation group was significantly higher than that in control group [94.1％ (32/34) vs 76.5％(26/34)] (P ＜ 0.05).After treatment,the LVEF and SV were significantly higher than those before treatment in observation group [(68 ±7)％ vs (45 ±6)％,(70 ±6) ml vs (51 ±5) ml] and control group [(53 ±5)％ vs (47±6)％,(57 ±6) ml vs (51 ±5) ml];they were significantly higher in observation group than those in control group (P ＜ 0.05).The pulmonary artery pressure and NT-proBNP after treatment were significantly lower than those before treatment in observation group [(42 ± 9) mmHg vs (80 ± 9) mmHg,(206 ± 72) ng/L vs (689 ± 198) ng/L] and control group [(54 ± 8) mmHg vs (78 ± 10) mmHg,(337 ± 102) ng/L vs (674 ±204) ng/L];they were significantly lower in observation group than those in control group (P ＜ 0.05).The LVEDDs were not significantly changed after treatment in both groups (P ＞ 0.05).No serious adverse reactions occurred in both groups.Conclusion Milrinone can effectively improve the heart function in patients with chronic pulmonary heart disease complicated with severe heart failure.
