Effect of staple replacement rich in dietary fiber on blood pressure in patients with type 2 diabetes mellitus

Abstract:Objective To explore the effect of 2 kinds of staple replacement rich in dietary fiber on blood pressure in patients with type 2 diabetes mellitus (T2DM).Methods Totally 107 cases with T2DM from October 2014 to January 2015 were enrolled and randomly divided into observation group (71 cases) and control group (36 cases).Before experiment,all patients were given high gluten wheat bread 50 g,eggs 50-55 g and milk 243 ml as breakfast.During experiment,control group was given wheat fiber bread instead of breakfast and dinner,each 50 g,observation group was given homology of medicine and food bread instead of breakfast and dinner,each 50 g;did not change daily intake of total calories,the experiment lasted for 12 weeks.The Liver and kidney function,body mass index (BMI),blood glucose,blood lipid and blood pressure before and after experiment were compared between groups.The BMI and blood pressure before and after experiment were compared between patients with and without hypertension.Results Totally 93 cases (62 cases in observation group and 31 cases in control group) completed the experiment,33 cases and 16 cases with hypertension in observation and control group respectively.After experiment,in observation group,the serum level of prealbumin was significantly higher than that before experiment [(277 ± 47) g/L vs (269 ± 49) g/L],the BMI,fasting blood glucose,1 and 2 h postprandial blood glucose (PBG),glycosylated hemoglobin (HbA1 c),high density lipoprotein cholesterol,low density lipoprotein cholesterol,systolic blood pressure (SBP) and diastolic blood pressure (DBP) were all significantly lower than those before experiment [(25 ± 3) kg/m2 vs (26 ± 3) kg/m2,(6.9 ± 1.2) mmol/L vs (7.5 ± 1.9) mmol/L,(11.2 ± 2.1) mmol/L vs (12.8 ± 3.2) mmol/L,(9.4 ± 2.5) mmol/L vs (12.1 ± 4.1) mmol/L,(6.3 ± 0.8) % vs (7.0 ± 1.2) %,(1.19 ± 0.24) mmol/L vs (1.27 ± 0.26) mmol/L,(2.7±0.9)mmol/L vs (2.9±1.0)mmol/L,(126±12)mmHgvs (130±22)mmHg,(73 ±10)mmHg vs (79 ± 13)mmHg];in control group,the BMI,2 h PBG and DBP were significantly lower than those before experiment [(26 ± 4) kg/m2 vs (27 ± 4) kg/m2,(10.0 ± 3.5) mmol/L vs (11.8 ± 3.8) mmol/L,(75 ± 10) mmHg vs (80 ± 9)mmHg];and the level of HbA1 c in observation group was significantly lower than that in control group [(6.3 ± 0.8) % vs (6.8 ± 1.0) %] (all P < 0.05).After experiment,the SBP and DBP were significantly lower than those before experiment in patients with hypertension in observation group [(132 ± 12) mmHg vs (144 ±20) mmHg,(74 ± 12)mmHg vs (83 ± 14)mmHg],the DBP was lower than that before experiment in control group [(78±11)mmHgvs (83±1O)mmHg,(71±9)mmHgvs (77±8)mmHg](allP<0.05).The BMIinpatients with hypertension in 2 groups were all significantly higher than that in patients without hypertension before and after experiment [observation group:(26.3 ± 3.7) kg/m2 vs (25.0 ± 2.7) kg/m2,(26.0 ± 3.6) kg/m2 vs (24.8 ± 2.4) kg/m2;control group:(27.9 ± 5.0) kg/m2 vs (25.1 ± 2.7) kg/m2,(27.5 ± 4.7) kg/m2 vs (24.7 ± 2.7) kg/m2] (all P < 0.05).Conclusion The homology of medicine and food bread has the same effect with wheat fiber bread,both can significantly reduce the BMI,2 h PBG and DBP in patients with T2DM.
