Influence of Shenfu injection on effect and endothelial function in patients with acute inferior wall ST-elevation myocardial infarction complicated with hypotension after percutaneous coronary intervention

Abstract：Objective To observe the influence of Shenfu injection on effect and endothelial function in patients with acute inferior wall ST-elevation myocardial infarction complicated with hypotension after percutaneous coronary intervention (PCI).Methods Totally 160 patients with acute inferior wall ST-elevation myocardial infarction complicated hypotension who underwent emergency PCI from May 2010 to February 2014 were randomly divided into conventional treatment group and combined treatment group (80 cases in each group).Both groups were given oral aspirin,clopidogrel and statin drugs and intravenous fluids,combined treatment group was additionally administered with Shenfu injection (50 ml,1 time/day,7 days).The systolic pressure,heart rate,levels of nitric oxide (NO),endothelin-1 (ET-1) and malondialdehyde (MDA) in plasma at admission,24 h and 7 d after treatment were compared between groups.Results The systolic pressure,heart rate,levels of NO,ET-1 and MDA in plasma were not significantly different between groups at admission (P ＞ 0.05).The systolic pressure,heart rate,plasma NO level in combined treatment group were significantly higher than those in conventional treatment group [(126 ±9) mmHg vs (105 ± 10) mmHg,(72 ±7) times/min vs (62 ±6) times/min,(81 ±12) μmol/L vs (72 ± 10) μmol/L] (P ＜0.05).The NO in plasma was significantly higher,the ET-1 and MDA in plasma were significantly lower in combined treatment group than those in conventional treatment group [(91 ±10) μmol/Lvs (82±11) μmol/L,(54±12) ng/L vs (65±11) ng/L,(6.9 ±0.9) μmol/L vs (8.4±0.6) μmol/L] (P ＜ 0.05).Conclusion Shenfu injection can effectively improve the blood pressure and heart rate,decrease lipid peroxidation damage,and protect the endothelial cells from damage in patients with acute inferior wall ST-elevation myocardial infarction complicated hypotension after PCI.
