Correlation between blood pressure and ischemic stroke in patients with uncontrolled hypertension

Abstract:Objective To analyze the correlations between ischemic stroke and systolic blood pressure (SBP),diastolic blood pressure (DBP),mean arterial pressure (MAP) in patients with uncontrolled hypertension.Methods In this cross-sectional study,subjects with uncontrolled hypertension aged above 35 years were recruited from the general medicine clinic of Beijing Anzhen Hospital and its affiliated community health centers from March to December 2014.The informations of gender,age,family history of hypertension,diagnosis of diabetes,current smoking and drinking,body mass index (BMI) and history of ischemic stroke were collected by unique questionnaire.The correlation between blood pressure and ischemic stroke was analyzed.Results Total 3 909 cases with uncontrolled hypertension were collected,among them,144 cases had ischemic stroke,with prevalence of of 3.7%.The prevalence of ischemic stroke was significantly different among patients with different SBP[<140 mmHg:1.5% (11/733),140-149 mmHg:2.6% (34/1 309),150-159 mmHg:5.9% (45/764),~160mmHg:4.9% (54/1 103)] and MAP [≤104 mmHg:3.1% (29/931),105-109 mmHg:2.6% (25/957),110-117 mmHg:3.5% (38/1 081),≥ 118 mmHg:5.5% (52/940)] (P <0.05);while the prevalence was not significantly different among patients with different DBP (P > 0.05).After adjustment of covariate factors,the risk increased by 1.769 and 1.105 folds in patients with SBP 150-159 mmHg and ≥ 160 mmHg compared with paitents of SBP < 140 mmHg,the risk increased by 0.708 fold in patients with MAP ≥118 mmHg compared with paitents of MAP ≤ 104 mmHg.After including SBP and MAP to the regression model,the risk increased by 1.334 in patients with SBP 150-159 mmHg compared with paitents of SBP < 140 mmHg (odds ratio:2.334,95% confidence interval:1.093-4.983,P < 0.05).In patients with SBP 150-159 mmHg,the risk of stroke decreased by 83.1% in MAP 105-109 mmHg compared with those of MAP ≤ 104 mmHg (odds ratio:0.169,95% confidenc interval:0.035-0.809,P < 0.05).Conclusion In patients with uncontrolled hypertension,SBP is significantly correlated with ischemic stroke;SBP > 150 mmHg is associated with a significantly increased risk of stroke;joint application of SBP and MAP is not superior to single application of SBP in predicting the risk of stroke;there is no significant correlation between stroke risk and DBP.
