Influence of creatinine level at discharge on prognosis of patients after cardiopulmonary bypass surgery

Abstract:Objective To investigate the influence of creatinine level at discharge on the prognosis of patients after cardiopulmonary bypass surgery.Methods Totally 1 905 patients ≥ 18 years old who underwent cardiac surgery with cardiopulmonary bypass surgery from July 1,2013 to May 31,2014 were retrospectively analyzed.The surgical type,preoperative complications,left ventricular ejective fraction,serum creatinine before and after surgery,duration of cardiopulmonary bypass and aortic clamping,and fluid balance during operation were recorded.All of the patients were followed up by telephone.Multivariate Cox regression model was used to analyze the influence factor of the prognosis.Results The mean age of 1 905 patients was (52 ± 14) years.Acute kidney injury (AKI) occurred in 403 patients (21.2%).In (10 ± 3) months of follow-up,55 cases (2.9%) died.The mortality increased with severity of AKI,ranging from 4.6% (15/327) for AKI stage 1 to 20.7% (6/29) for AKI stage 3.The mortality in patients with creatinine level at discharge ≥88.4 μmol/L was significantly higher than that in patients with creatinine level at discharge < 88.4 μmol/L (0.21% vs 0.03%) (P < 0.001).High creatinine level at discharge remained the independent predictor of death (relative risk:1.010,95 % confidence intervals:1.00 1-1.014,P < 0.001).Conclusion High creatinine level at discharge was an independent predictor of death in patients undergoing cardiopulmonary bypass surgery.
