Clinical significance of serum brain natriuretic peptide level in elderly and non-elderly patients with persistent atrial fibrillation

Abstract:Objective To explore the clinical significance of serum brain natriuretic peptide (BNP) level in elderly and non-elderly patients with persistent atrial fibrillation.Methods Totally 246 patients with persistent atrial fibrillation from January 2009 to December 2013 were enrolled and divided into elderly group (≥65 years old,62 cases) and non-elderly group (<65 years old,184 cases).The laboratory biomarkers [BNP,high sensitive C reactive protein (hs-CRP),homocysteine (Hcy),D-dimer],echocardiography [left ventricular ejection fraction (LVEF),left atrial diameter (LAD)] and hospitalization time were compared between two groups.Results The elderly group had higher levels of BNP and D-dimer compared with non-elderly group [211 (145,350) ng/L vs 127 (78,222) ng/L,90 (51,144) μg/L vs 46 (24,79) μg/L] (P <0.01).The levels of hs-CRP,Hcy,LVEF,LAD and hospitalization time were not significantly different between groups (all P >0.05).BNP was positively correlated with D-dimer,LAD and hospitalization time (r =0.292,0.343,0.207,P < 0.01),while was negatively correlated with LVEF (r =-0.219,P < 0.05);BNP was not correlated with hs-CRP and Hcy (r =0.089,0.035,P > 0.05).Conclusion The BNP level in elderly patients with persistent atrial fibrillation was significantly higher than those in non-elderly patients,BNP may be used to evaluate the thrombotic state and cardiac function status,but not inflammatory reaction.
