Effect of intensive insulin therapy on stress hyperglycemia and risk of hypoglycemia in acute cerebral stroke: Meta-analysis

Abstract:Objective To evaluate the regulation effect of intensive insulin therapy on stress hyperglycemia and risk of hypoglycemia in acute cerebral stroke using Meta-analysis.Methods The randomized control trials (RCT) of intensive insulin therapy (intensive group) and conventional insulin therapy (conventional group) in acute cerebral stroke complicated with stress hyperglycemia were retrieved.The Jadad scale was applied to evaluate the quality of included references;Meta analysis was performed to analyze the blood glucose and incidence of hypoglycemia with RevMan 5.2 software.Results Seven RCTs were included in this study;the level of blood glucose was analyzed in 5 RCTs and the incidence of hypoglycemia was analyzed in 4 RCTs.Meta-analysis showed that the mean glucose level in intensive group was significantly lower than that in conventional group [mean difference :-2.52, 95% confidence interval :-4.30--0.74, P =0.006];the incidence of hypoglycemia was not significantly different between groups (risk ratio: 2.66, 95% confidence interval: 0.91-7.78, P =0.07).Conclusion Intensive insulin therapy shows better efficacy than conventional insulin therapy in regulating stress hyperglycemia in patients with acute cerebral stroke, but without increasing the risk of hypoglycemia.
