Correlation between severity of coronary lesion and renal function in patients with acute coronary syndrome

Abstract:Objective To investigate the correlation between severity of coronary lesion and renal function in patients with acute coronary syndrome (ACS).Methods A total of 209 patients with ACS who underwent coronary angiography were enrolled and divided into normal renal function group [estimated glomerular filtration rate (eGFR)≥90 ml/(min · 1.73 m2), 65 cases], mild renal dysfunction group [eGFR 60-89 ml/(min · 1.73 m2),115 cases] and moderate/severe renal dysfunction group [eGFR < 60 ml/(min · 1.73 m2), 29 cases].The severity of coronary lesion was assessed by Gensini score.The correlation between eGFR and the severity of coronary lesion was analyzed.The patients were divided into single/double lesion group and three/left main lesion group.The clinical characteristics of the two groups were compared.Results The proportion of severe coronary lesions and Gensini score in mild renal dysfunction group and moderate/severe renal dysfunction group were significantly higher than those in normal renal function group [78.3% (90/115), 75.9% (22/29) vs 53.8% (35/65);(75 ± 42), (78 ± 37) scores vs (48 ± 29) scores] (all P < 0.05).The proportions of diabetes and hypertension in group B were significantly higher than those in group A [61.2% (90/147) vs 23.7% (14/59), 78.2% (115/147) vs 55.9% (33/59)];the the level of hemoglobin A1c (HbA1c) and Gensini score in group B were significantly higher than that in group A [(7.5 ± 1.6) % vs (6.3 ± 0.9) %;(77 ± 39) scores vs (43 ± 30) scores] (all P < 0.05).The eGFR in group B was significantly lower than that in group A [(77 ± 17) ml/(min · 1.73 m2) vs (85 ±21) ml/(min · 1.73 m2)] (P <0.05).The Gensini score was positively correlated with age (r =0.278, P < 0.001), body mass index (r =0.170, P =0.015) and HbA1c (r =0.385,P < 0.001), was negatively correlated with left ventricular ejection fraction (r =-0.227, P =0.001) and eGFR (r =-0.362, P < 0.001).Logistic regression analysis indicated that renal dysfunction (odd ratio =3.093, 95% confidence interval: 1.599-5.984, P =0.001) and HbA1 c (odd ratio =1.930, 95% confidence interval: 1.300-2.866, P =0.001) were independent risk factors of severe coronary lesions.Conclusions The severity of coronary lesion is correlated with eGFR.Renal dysfunction and HbA1c are independent risk factors of severe coronary artery disease.
