Value of coagulation function in assessing disease condition and prognosis of patients with early traumatic injury

Abstract：Objective To explore the value of coagulation function in assessing disease condition and prognosis of patients with early traumatic injury.Methods Totally 200 patients with traumatic injury from January 2011 to January 2014 were divided into mild group [record chronic health evaluation and acute physiology (APACHE Ⅱ) score ＜20 scores, 160 cases] and severe group (APACHE Ⅱ score ≥20 scores, 40 cases)according to APACHE Ⅱ score, survival group (155 cases) and death group (45 cases) according to the prognosis within 14 d after admittion in ICU.The blood coagulation indicators including platelet count (Plt), plateletcrit (PCT), platelet large cell ratio (PLCR), platelet the average volume (MPV), platelet distribution width (PDW), prothrombin time (PT), prothrombin activity (PTA), activated partial thromboplastin time (APTT),fibrinogen (FIB), prothrombin time (TT), two D-dimer and serum calcium concentration (Ca2+) were compared between groups.Results The Plt, FIB, PCT, PTA were significantly lower, the PLCR, MPV, PDW, PT,TT, APTT and D-dimer were significantly higher/longer in severe group than those in mild group [(105 ±56) ×109/Lvs (151±92) ×109/L, (1.8±1.2) g/Lvs (2.4±1.6) g/L, (0.12±0.08)％ vs (0.18±0.11)％,(64±19)％ vs (77±16)％, (37±10)％ vs (33±8)％, (12.9±1.3) fl vs (11.0±1.1) fl, (15.4±3.1)％ vs (12.2±2.4)％, (16.5±5.0) s vs (13.1 ±2.7) s, (26±3) s vs (17 ±4) s, (55 ±31) s vs (43 ± 19) s, (1 659 ±599) μg/L vs (875 ±263) μg/L] (P ＜0.05).The PT was significantly longer, the PTA was significantly lower, the D-dimer was significantly higher in death group than those in survival group [(16.1 ±5.0) s vs (13.3 ±2.7) s, (62 ± 19)％ vs (72 ± 17)％, (2 267 ± 1 023) μg/L vs (872 ±746) μg/L](P ＜ 0.05).Conclusion The blood coagulation function is damaged in patients with early traumatic injury, and the hypocoagulability can reflect the disease condition and the prognosis.
