Analysis of surface electrocardiogram of dual atrio-ventricular nodal pathways with 1 : 2 conduction

Abstract:Objective To analyze the characteristics of surface electrocardiogram (ECG) of dual atrioventricular (AV) nodal pathways with 1 ∶ 2 conduction.Method ECG data of 9 patients of dual AV nodal pathways with 1 ∶ 2 conduction were retrospectively analyzed.Results There were 4 cases with sinus rhythm showing 1 ∶ 2 AV conduction;2 cases with sinus rhythm showing alternative 1 ∶ 1 and 1 ∶ 2 AV conduction;1 case with sinus rhythm showing 1 ∶ 1 AV conduction along the slow pathway or 1 ∶ 2 AV conduction, occasionally 1 ∶ 1 AV conduction along the fast pathway;1 case with non-paroxysmal atrial tachycardia showing 1 ∶ 2 AV conduction, occasionally 1 ∶ 1 AV conduction along the fast pathway, 1 ∶ 0 AV conduction once;1 case with sinus rhythm showing AV couduction along the fast and slow pathway with a stable cycle, manifesting as pseudo Wenckebach's phenomenon.Conclusion Dual AV nodal pathways with 1 ∶ 2 conduction is rare in surface ECG, mostly being sinus rhythm;this study reports 1 case with non-paroxysmal atrial tachycardia showing 1 ∶ 2, occasionally 1 ∶ 0 AV conduction.
