Risk factors of acute pulmonary thromboembolism in elderly and the non-elderly: a comparison analysis

Abstract:Objective To compare the clinical risk factors of acute pulmonary thromboembolism (PTE) in the elderly and non-elderly.Methods Totally 82 patients with acute PTE from October 2013 to October 2014 were retrospectively analyzed.The patients were divided into elderly group (≥ 65 years old,40 cases) and non-elderly group (< 65 years old,42 cases).The clinical manifestations,blood gas analysis,acquired and genetic risk factors were analyzed and compared between groups.Results All the patients (100.0%) had chest tightness and shortness of breath after activities;chest pain and syncope were the main clinical manifestations in elderly group,chest pain and hemoptysis were the main clinical manifestations in non-elderly group.Hypoxemia,hypocarbia and increased alveolar-arterial oxygen were observed in both groups (P > 0.05).For acquired risk factors,the deep vein thrombosis of lower limbs was the most common in both groups,without significant difference [70.0% (28/40) vs 54.8% (23/42)] (P > 0.05);brake (limited mobility),malignant tumor and cardiopulmonary diseases were more common in elderly group compared with non-elderly group [45.0% (18/40) vs 14.3% (6/42),25.0% (10/40) vs7.1% (3/42),82.5% (33/40) vs47.6% (20/42)] (P<0.05).There were no statistical differences regarding the genetic risk factors between the two groups (P > 0.05).Conclusions PTE in elderly patients tends to be misdiagnosed and missed because of untypical clinical manifestations and consolidated basic cardiopulmonary diseases;acquired risk factors should be actively interuencd to reduce the morbidity of PTE in elderly patients.
