Comparison of lamivudine combined with adefovir dipivoxil and entecavir combined with adefovir dipivoxil in chronic hepatitis B patients with adefovir dipivoxil resistance

Abstract:Objective To evaluate and compare the effect of lamivudine (LAM) combined with adefovir dipivoxil (ADV) and entecavir (ETV) combined with ADV in chronic hepatitis B patients with ADV resistance.Methods Seventy-six CHB patients with ADV resistance were randomly divided into LAM + ADV group (38 cases) given oral administration of LAM(100 mg/d) and ADV(10 mg/d) for 96 weeks,and ETV + ADV group (38 group) given oral administration of ETV (0.5 mg/d) and ADV (10 mg/d) for 96 weeks.The negative rate of hepatitis B virus (HBV) DNA,normalization rate of alanine aminotransferase (ALT),seroconversion rate of HBV e antigen (HBeAg) and virological breakthrough 12,24,48 and 96 weeks were observed and compared between the two groups.Results After 96 weeks of treatment,the negative rate of HBV DNA and normalization rate ofALTin LAM + ADV group were both higher than those in ETV + ADV group [92.1% (35/38) vs 73.6% (28/38),89.5% (34/38)vs71.1% (27/38)] (x2 =4.547 and4.070,both P<0.05).No significant difference of HBeAg seroconversion rate was shown between the two groups (P > 0.05).No virological breakthrough occurred in ETV + ADV group during 96 weeks of treatment;there was one patients had virological breakthrough in LAM + ADV group.No severe adverse reactions occurred in both groups.Conclusion The effect of LAM combined with ADV is better in terms of inhibiting virus replication and reducing drug resistance,compared with ETV combined with ADV in chronic hepatitis B patients with ADV resistance;however it does not improve the HBeAg seroconversion rate.
