Safety and feasibility of respiratory function scores for determination of weaning time of mechanical ventilation

Abstract:Objective To investigate the safety and feasibility of respiratory function scores (RFS) for determination of weaning time of mechanical ventilation in patients with respiratory failure.Methods Totally 100 patients with acute exacerbation of chronic obstructive pulmonary diseases (COPD) who had respiratory failure and underwent mechanical ventilation were enrolled and divided into traditional withdrawal group (50 cases),RFS3-4 score group (32 cases) and RFS5-6 score group (18 cases).The situation of ventilator weaning,intensive care unit ward stay and incidence of complications were compared between the two groups.Results The direct weaning success rate in RFS 3-4 withdrawal group was significantly higher than that in RFS 5-6 withdrawal group [96.9% (31/32) vs 83.3% (15/18),P =0.02].Duration of mechanical ventilation and length of intensive care unit ward stay in RFS 3-4 withdrawal group and RFS 5-6 withdrawal group were all significantly lower than those in traditional withdrawal group [(5.2 ±2.5),(5.0 ±3.0) d vs (9.0 ±4.0) d,(8.1 ± 1.9),(9.1 ±2.1) d vs (13.2 ± 3.0) d] (P < 0.05).The incidences of ventilator associated pneumonia and pulmonary barotrauma in RFS 3-4 withdrawal group and RFS 5-6 withdrawal group were significantly lower than those in traditional withdrawal group [15.6% (5/32),16.7% (3/18) vs26.0% (13/50),9.4%(3/32),5.5% (1/18) vs 14.0% (7/50)](P< 0.05).Conclusion The respiratory function scores is safe and feasible for the determination the ventilation weaning time in treatment of respiratory failure,RFS3-4 score can be set as time window of mechanical ventilation weaning.
