Cross-sectional study on metabolic disorder and target organ damage in young patients with hypertension

Abstract:Objective To explore etiology,risk factors and target organ damages in young patients with hypertension.Methods A total of 203 young patients with hypertension from January 2008 to December 2012 were enrolled.The disease history,family history,height,weight,office blood pressure,fasting plasma glucose (FPG),triglyceride (TG),total cholesterol (TC),high density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),blood urea nitrogen (BUN),creatinine (CR) and serum uric acid (SUA) were collected.The data of 24-hour ambulatory blood pressure monitoring,polysomnography (PSG) and transthoracic echocardiography were also recorded.Results There were 172 cases of essential hypertension and 31 of secondary hypertension.The proportion of male was higher than that of female in patients with essential hypertension [88.7% (118/133) vs 77.1% (54/70)],was lower than that of female in secondary hypertension patients [11.3% (15/133) vs 22.9% (16/70)] (P < 0.05).Among patients with essential hypertension,128 (74.4%) patient had more than one kind of metabolic abnormality,mainly including abnormal lipid metabolism [92 (53.5%)] and obesity [73 (42.4%)].The most common target organ damage was left ventricular hypertrophy [47 (23.7%)].Among the essential hypertension patients,the males had higher body mass index,CR,uric acid,24 h-systolic blood pressure and incidence of left ventricular hypertrophy,but lower HDL-C compared with the females [(28 ±5) kg/m2 vs (26 ±4) kg/m2,(86 ±23) μmol/L vs (67 ±28) μmol/L,(436 ±114) μmol/L vs (326±89) μmol/L,(134 ± 16) mmHg vs (128 ±16) mmHg,29.9% (35/118) vs 14.8% (8/54),(1.1 ±0.2) mmol/L vs (1.2 ±0.4) mmol/L] (all P <0.05).There were no significant differences in biochemical indices and target organ damage between the cases with positive and negative family history in patients with essential hypertension (P < 0.05).Conclusion Essential hypertension is predominantly in the young inpatients with hypertension,especially in the male patients;metabolic disorders are common in them,which should be improved to reduce the damages of the target organs.
