Value of left atrial volume index in predicting major adverse cardiovascular events in unstable angina patients

Abstract:Objective To investigate the value of left atrial volume index (LAVI) in predicting the major adverse cardiovascular events (MACE) in unstable angina patients.Methods Totally 178 patients with unstable angina from January 2012 to September 2013 were enrolled and divided into group A (LAVI ≥ 32 ml/m2,86 cases) and group B (LAVI < 32 ml/m2,92 cases).The clinical data and echocardiography indices were compared between the two groups.All patients were followed up for 12 months to observe the major adverse cardiovascular events (MACE) and the independent risk factors were analyzed.Results The left ventricular end diastolic diameter (LVEDD),left ventricular end-systolic diameter (LVEDS),pulmonary vein orifice Ar peak,mitral valve orifice E peak,left atrial area on four chamber and two chamber view were larger,the anteroposterior diameter and vertical diameter of left atrial diameters were shorter in group A compared with those in group B [(55 ±4) mm vs (48 ±5) mm,(35 ±5) mm vs (29 ±6) mm,(27 ±6)cm/s vs (25 ±6)cm/s,(78 ±20)cm/s vs (73 ± 16)cm/s,(22.9±0.9) mm2 vs (22.0±1.3) mm2,(21.8±1.1) mm2 vs (20.7±1.4) mm2,(44±3) mmvs (47± 5) mm,(31 ±4) mm vs (33 ±4) mm] (all P <0.05).The proportion of hypertension and diabetes,levels of creatine kinase (CK) and CK-MB,the proportion of patients receiving percutaneous coronary intervention (PCI) were higher[64.0% (55/86) vs 58.7% (54/92),66.3% (57/86) vs 63.0% (58/92),(172 ±35)U/L vs (83 ± 33)U/L,(19±6)U/L vs (13±4)U/L,69.8% (60/86) vs 66.3% (61/92)],the weight and white blood cell count were lower in group A compared with those in group B[(50 ±8)kg vs (60 10)kg,(9.3 ±3.2) × 109/L vs (10.3 ± 3.3) × 109/L] (all P < 0.05).During 12-month follow-up,MACE occurred in 70 cases,including 53 cases of group A and 17 cases of group B.The LAVI,LVEDD,LVEDS,CK,CK-MB,the proportion of diabetes were higher,the proportion of patients receiving PCI was lower in MACE patients compared with those in non-MACE patients [(36.3 ±2.6) ml/m2 vs (28.7 ±3.3) ml/m2,(53.4 ± 1.9) mm vs (49.9 ±2.1) mm,(36.7 ± 1.6) mm vs (34.8±1.5)rmm,(199±14) U/Lvs (70±11) U/L,(19±7) U/Lvs (9±3) U/L,80.0% (56/70) vs 38.9% (42/108),68.6% (48/70) vs 80.6% (87/108)] (all P < 0.05).The independent risk factors in predicting MACE included LAVI (OR =1.217,95% CI:1.116-1.318,P < 0.05),CK (OR =2.126,95% CI:1.196-3.056,P<0.01) and CK-MB (OR =1.968,95%CI:1.168-2.768,P<0.01).The LAVI was positively correlated with CK and CK-MB (r =0.739,r =0.477,P < 0.01).Conclusion LAVI is positively correlated with CK and CK-MB,which is an independent predictor for MACE in patients of unstable angina.
