Correlation between stress hyperglycemia and prognosis of acute intracerebral hemorrhage

Abstract:Objective To investigate the correlation between stress hyperglycemia and the prognosis of acute intracerebral hemorrhage.Methods A total of 181 patients with acute intracerebral hemorrhage from January to June 2014 were recruited and divided into normal blood glucose group [fasting blood glucose (FBG) <7.0 mmol/L) and stress hyperglycemia group (FBG ≥7.0 mmol/L) which was further divided into low level (FBG 7.0-10.0 mmol/L),middle level (FBG 10.1-15.0 mmol/L) and high level (FBG > 15.0 mmol/L) subgroup.The clinical data,Glasgow coma scale (GCS),localization of bleeding,amount of hemorrhage and complications were compared between normal blood glucose group and stress hyperglycemia group.The Glasgow outcome scale was used to evaluate the prognosis 30 d after the onset.The correlation between the blood glucose and prognosis was assessed and the risk factors of the poor prognosis were analyzed.Results The GCS score and the amount of bleeding were significantly different between normal blood glucose group and stress hyperglycemia group [GCS 13-15 scores:71.6% (58/81) vs 37.0% (37/100),9-12 scores:17.3% (14/81) vs 14.0% (14/100),≤8 scores:11.1% (9/81) vs 49.0% (49/100);amount of bleeding ≥30 ml:19.8% (16/81) vs 37.0% (37/100),< 30 ml:80.2% (65/81) vs 63.0% (63/100)] (all P < 0.05),while the gender,age,medical history and localization of bleeding were not different (all P > 0.05).The rate of poor prognosis in normal blood glucose group and low level,middle level,high level subgroup of stress hyperglycemia was 29.6% (24/81),52.4% (22/42),67.5% (27/40) and 83.3% (15/18),respectively,which was increased along with raise of the blood glucose level (P < 0.01),the blood glucose level was negatively correlated with prognosis (rs =-0.475,P < 0.05).Multivariate Logistic regression showed that the risk factors of the poor prognosis in acute intracerebral hemorrhage patients included low GCS score [9-12 scores:OR =4.51 (95% CI 1.41-14.45),≤ 8scores:OR =61.65 (95% CI 14.81-256.68)],stress hyperglycemia [OR =4.22 (95% CI 2.25-7.99)],amount of bleeding≥30 ml [OR =8.47 (95% CI 2.40-29.87)] and complication[OR =5.18 (95% CI 1.41-19.03)] (all P < 0.01).Conclusions Stress hyperglycemia can be used as an indicator for assessing the severity of condition and predicting prognosis in patients with acute intracerebral hemorrhage.Monitoring and controlling the blood glucose level early are beneficial for improving the prognosis of acute intracerebral hemorrhage.
