Effect of tolvapta in intractable heart failure complicated with hyponatremia in elderly patients

Abstract:Objective To investigate the clinical efficacy of tolvapta in intractable heart failure complicated with hyponatremia in elderly patients.Methods One hundred and fifty elderly patients (≥65 years old) with intractable heart failure complicated with hyponatremia were enrolled and administrated with tolvaptan (15-60 mg/d) in addition to conventional therapy.Before and 7 days after treatment,the levels of blood sodium,brain natriuretic peptide (BNP),hypersensitive C-reactive protein (hs-CRP) and plasma osmotic pressure were measured and compared.Results Seven days post tolvaptan therapy,the levels of plasma sodium and plasma osmotic pressure were increased significantly,and the level of plasma BNP and plasma hs-CRP were decreased [(138 ± 17) mmol/L vs (112±15) mmol/L,(273 ±44) mOsm/L vs (196±30) mOsm/L,(1463±326) g/L vs (2538±460) μg/L,(13.7 ± 0.9) mg/L vs (17.8 ± 1.2) mg/L] (P < 0.05).Left ventricular end-diastolic diameter and left ventricular ejection fraction showed no significant changes after treatment compared with those before treatment (P > 0.05).Conclusion Tolvaptan can correct the hyponatremia and reduce plasma BNP level in elderly patients with heart failure.
