Effect of telbivudine and lamivudine in chronic hepatitis B complicated with decompensated cirrhosis

Abstract：Objective To evaluate the efficacy of telbivudine and lamivudine in treating chronic hepatitis B complicated with decompensated cirrhosis.Methods Ninety four hepatitis B patients with decompensated liver cirrhosis from August 2012 to August 2014 were enrolled and randomly divided into telbivudine group and lamivudine group.The negative conversion rate of serum hepatitis B virus (HBV) DNA,YMDD mutation,Child-Pugh integral,total bilirubin,albumin,prothrombin time and adverse reaction were evaluated before and 12,24,48 weeks after treatment.Results The negative conversion rate of HBV-DNA and Child-Pugh integral 12,24,48 weeks after treatment showed no significant differences between two groups (all P ＞ 0.05).No YMDD mutation was found in telbivudine group;it was found in 15.4％ (8/52) in lamivudine group after 48 weeks of treatment.In telbivudine group,liver functions related indices including total bilirubin,albumin and prothrombin time were all significantly improved after 12,24,48 weeks of treatment[(51 ± 10),(42 ±5),(36 ±3) μmol/L vs (56 ± 48) μmol/L,(28 ±4),(33 ±3),(33 ±4) g/L vs (27 ±5) g/L,prothrombin time:(15 ±5),(16 ±5),(15 ±4) s vs (17 ± 9) s,all P ＜ 0.05].In lamivudine group,total bilirubin was improved after 12 weeks of treatment [(53 ±9) μmol/L vs (43 ± 8) μmol/L];albumin was improved after 12,24 and 48 weeks of treatment [(31 ±4),(33 ±4),(33 ±3) g/L vs (27 ±7) g/L];prothrombin time was siguificantly improved after 12 and 48 weeks of treatment [(16 ± 6),(16 ± 4) s vs (17 ± 8) s] (all P ＜ 0.05].No obvious adverse reactions occurred in all patients.Conclusion Telbivudine and lamivudine are both effective in inhibiting virus replication and improving of liver function in chronic hepatitis B patients with decompensated cirrhosis,but lamivudine can induce YMDD mutation.
