Effect of different doses of ulinastatin on extravascular lung water index in patients of septic shock

Abstract:Objective To evaluate the effect of different doses of ulinastatin on the extravascular lung water index (EVLWI) in patients of septic shock.Methods Ninety patients with septic shock admitted to intensive care init (ICU) were divided into control group receiving conventional therapy (30 cases),group A (30 cases) receiving 0.4 × 104 U/kg ulinastatin (1 time/12 h) based on conventional therapy and group B (30 cases) receiving 0.8 × 104 U/kg ulinastatin (1 time/12 h) based on conventional therapy.EVLWI was monitored and according to the EVLWI level,the patients of each group were divided into EVLWI 0-7 ml/kg,8-14 ml/kg and > 14 ml/kg subgroup,named as control group 1,control group 2 and control group 3,group A1,group A2 and group A3,group B1,group B2 and group B3.The different doses of ulinastatin was dissolved in 50 ml of 0.9% sodiumchloride injection saline and injected using continuous micro-pump with speed of 25 ml/h for 7 d.The levels of EVLWI,respiratory index(RI),pulmonary vascular permeability index (PVPI) and cardiac index (CI) at different time (0,24,48,72 h after admission to ICU) were measured in each group to observe the effect of ulinastatin.Results ①The general condition,acute physiology and chronic health evaluation Ⅱ score of the patients showed no statistical differences among group A,B and control group (all P > 0.05).Compared with admission,the levels of EVLWI,RI and PVPI dropped significantly at 48 h in group B (P < 0.05),and at 72 h in group A and B (P < 0.05).The levels of EVLWI,RI and PVPI in group B were obviously lower than those of control group and group A at 48 h (P < 0.05),and the levels of above indices in group A and group B were significantly lower than those of control group at 72 h(P < 0.05 or P < 0.01).②Compared with admission,the levels of EVLWI,RI and PVPI decreased significantly at 48 h and 72 h in group A2 and B2 (P < 0.05 or P < 0.01),at 72 h in group B3 (P < 0.05).The levels of EVLWI,RI,PVPI and CI in the other subgroups showed no statistical changes compared with admission at any time.The levels of EVLWI,RI and PVPI in group A2 and B2 were significantly lower than those in control group 2 at 48 h and 72 h (P < 0.05 or P < 0.01),and the levels of above indices in group B3 were significantly lower than those of control group 3 and group A3 at 72 h (P < 0.05).Conclusions Ulinastatin can decrease EVLWI and PVPI,and it can improve the respiratory function in patients of septic shock.
