Clinical application of gasless laparoscopy in myomectomy

Abstract:Objective To compare the feasibility,safety,reliability and postoperative outcomes of myomectomy perferformed by gasless laparoscopy and conventional laparoscopy.Methods Forty-two women with symptomatic uterine myomas were randomly divided randomly into gasless laparoscopy group (gasless group,20 cases) and conventional laparoscopy group (conventional group,22 cases).All procedures were performed by same surgeons.Blood gas analysis at three time(before the operation,one hour during the operation and after the operation),surgical duration,blood loss,sign of tumor,drainage volume from abdominal cavity,immediate postoperative outcomes and discharge time were recorded.Results All of the operations had been completed successfully.compared with the gasless laparoscopy group,during the gasless laparoscopy myomectomy,the PaCO2 and BE rised [partial pressure of carbon dioxide (PaCO2):(44.3 ± 3.2) mmHg vs (41.3 ± 1.3),(42.6 ± 4.2) mmHg ; base excess (BE):[(-7.4 ± 0.9) mmol/L vs (-3.2 ± 3.4),(-4.2 ± 0.8) mmol/L,P < 0.05],Partial pressure of carbon dioxide,base excess,partial pressure of oxygen,oxygen saturation,pH value,systolic pressure,diastolic pressure of intraoperative and preoperative,postoperative had no statistical significance in gasless laparoscopy group (P > 0.05).Conclusion Twol of the operations had been completed successfully.Gasless laparoscopy have more advantage.
