The relationship between clopidogrel resistance and insulin resistance in patients with acute ST-segment elevation myocardial infarction

Abstract:Objective To investigate the relationship between clopidogrel resistance and type 2 diabetes in patients with acute ST-segment elevation myocardial infarction.Methods A total of 978 patients with acute ST-segment elevation myocardial infarction at Beijing Anzhen Hospital were enrolled in the study.The number of patients with diabetes mellitus and without diabetes mellitus was 272 cases (27.8%) and 706 cases (72.2%) respectively.Clopidogrel resistance was defined as the rate of platelet aggregation of more than 50% induced by 5 μmol/L adenosine diphosphate.Combined endpoint events and the incidence of clopidogrel resistance were observed.Results Incidence of clopidogrel resistance was significantly higher in type 2 diabetes group than in nontype 2 diabetes group [44.1% (120/272) vs 14.7% (104/706),P =0.017].Combined end point event rate was significantly higher in the type 2 diabetes group than in non diabetes group[16.2% (47/272) vs 7.4% (52/706),P =0.012].Logistic regression analysis showed that type 2 diabetes was an independent predictor of clopidogrel resistance in acute ST-segment elevation myocardial infarction (OR:4.710,95% CI:1.380-7.551,P =0.028).Conclusions Combined endpoint events and the incidence of clopidogrel resistance are higher in type 2 diabetes patients with acute ST-segment elevation myocardial infarction than in non diabetes patients with acute ST-segment elevation myocardial infarction.Type 2 diabetes is an independent predictor of clopidogrel resistance in acute ST-segment elevation myocardial infarction.
