Comparison of intermittent and continuous non-invasive mechanical ventilation in severe respiratory failure patients

Abstract:Objective To observe the effects of intermittent and continuous non-invasive mechanical ventilation in severe respiratory failure patients.Methods From January 2011 to December 2013 in Beijing Geriatric Hospital,85 hospitalized patients who suffered fromsevere respiratory failure due to acute exacerbation of chronic obstructive pulmonary disease were randomly divided into observation group 1 (28 cases),observation group 2 (28 cases) and control group (29 cases).On the basis of routine treatment,the control group was given continuous noninvasive mechanical ventilation therapy; the observation group 1 received 3 hours of noninvasive mechanical ventilation and 1 hour nasal catheter oxygen; the observation group 2 received 2 hours of noninvasive mechanical ventilation and two hours nasal catheter oxygen.Three groups were observed for 72 hours and blood gas analysis before treatment and was detected.Results Among the three groups,pH value,arterial partial pressure of oxygen (PaO2) and arterial partial pressure of carbon dioxide (PaCO2) were not statistically different before treatment (P > 0.05) ; but they were statistically different from those 4,12,24,48,72 h after treatment [observation group 1:pH value were (7.27 ±0.04),(7.30 ±0.04),(7.33 ±0.03),(7.37 ±0.04),(7.39 ± 0.04) vs (7.22 ± 0.04),PaO2 were (58 ± 8) mmHg (1 mmHg =0.1 33 kPa),(64 ± 8) mmHg,(70 ± 7) mmHg,(74 ± 7) mmHg,(80 ±7)mmHg vs (48 ±6) mmHg,PaCO2 were (75 ±6)mmHg,(70 ±6)mmHg,(64 ±7) mmHg,(58 ±7) mmHg,(51 ± 6) mmHg vs (82 ± 7) mmHg; observation group 2:pH value were (7.25 ± 0.03),(7.30 ± 0.03),(7.33 ± 0.03),(7.36 ±0.03),(7.38 ±0.04)vs (7.22 ±0.04),PaO2 were (57 ± 8) mmHg,(63 ±8) mmHg,(63 ± 7) mmHg,(73 ± 7) mmHg,(79 ± 7) mmHg vs (48 ± 5) mmHg,PaCO2 were (76 ± 6) mmHg,(71 ± 5) mmHg,(65 ± 6) mmHg,(59 ± 7) mmHg,(52 ± 6) mmHg vs (83 ± 7) mmHg; control group:pH value were (7.26 ± 0.04),(7.30 ±0.04),(7.34 ±0.04),(7.37 ±0.04),(7.39 ± 0.03) vs (7.21 ±0.04),PaO2 were (60 ± 9) mmHg,(65 ± 8) mmHg,(71 ± 7) mmHg,(75 ± 7) mmHg,(80 ± 7) mmHg vs (48 ± 6) mmHg,PaCO2 were (76 ± 6) mmHg,(70 ± 5) mmHg,(63 ± 6) mmHg,(57 ± 7) mmHg,(50 ± 6) mmHg vs (83 ± 6) mmHg; all P < 0.01].With the extension of treatment duration,pH,PaO2 and PaCO2 were continuously improved and the differences were statistically significant(all P < 0.01).The observation group l and group 2 were higher than the control group regarding improvement rate of cough and reduced rate of sputum [96.4% (27/28),85.7% (24/28) vs 62.1% (18/29) ; 92.8% (26/28),78.6% (22/28) vs 51.7% (15/29),all P < 0.05],and the difference was not statistically significant between the observation group 1 and group 2(P > 0.05).The observation group 1 and group 2 were lower than the control group regarding the incidence rates of facial sores and flatulence [10.7% (3/28),7.1% (2/28) vs 34.5% (10/29) ; 28.6% (8/28),21.4% (6/28) vs 69.0% (20/29) ; all P <0.05],and the difference was not statistically significant between the observation group 1 and group 2 (P > 0.05).Conclusion The treatment effect of two intermittent noninvasive mechanical ventilation is similar to that of continuous noninvasive mechanical ventilation but with less cough,sputum,better facial pressure sores and flatulence improvement.
