Effect of mild hypothermia therapy combined with erythropoietin on expression of amyloid-β protein in patients with severe traumatic brain injury

Abstract:Objective To investigate the effect of mild hypothermia therapy (MHT) combined with erythropoietin (EPO) therapy on the expression of amyloid-β (Aβ) protein in the cerebrospinal fluid (CSF) of patients with severe traumatic brain injury (sTBI).Methods One hundred and twenty patients with sTBI were divided into four groups:the control group,the MHT group (patients were cooled to 32-35 ℃ for 1-7 days),the EPO group (patients were given 75-100 IU/kg of EPO intravenously for 3 times per week),the MHT combined with EPO (MHT + EPO) group.On the basis of conventional therapy,patients were treated with MHT,or EPO,or MHT combined with EPO during the same course of treatment.The level of Aβ protein in the CSF was determined by enzyme-linked immunosorbent assay (ELISA).The scores of Glasgow Coma Scale (GCS) after 3 months of treatment,the glasgow outcome scale (GOS) scores and long-term quality of Life Assessment Scale (KPS) scores after 6 months of treatment were observed,respectively.Results There were no missing cases within 7 d following TBI-treatment.After the 1,3 and 7 day of treatment,the ICP values and the level of Aβ protein for the patients were decreased obviously in the MHT,EPO,and MHT + EPO group.Furthermore,the intracranial pressure values and the level of Aβ protein in the MHT + EPO group were lower than those of the other 3 groups (P <0.01).After 3 months of treatment,the scores of GCS of the MHT + EPO group were significantly higher than those of the other3 groups [(11.1±1.4)scores vs (7.6±1.1),(9.5±1.4),(9.5±0.9)scores] (P<0.01).After 6 months of treatment,the scores of GOS [(4.3 ± 0.7) scores vs (3.1 ± 1.0),(3.8 ± 1.0),(3.8 ± 0.8) scores]and KPS [(76 ± 14) scores vs (54 ± 11),(63 ± 16),(64 ± 13) scores] of the MHT + EPO group were also significantly higher than those of control group,MHT group and EPO group (P < 0.01).Conclusion Mild hypothermia combined with EPO therapy can inhibit the level of Aβ protein in the CSF and it can improve the neurological function and considerably improve prognosis of patients with severe traumatic brain injury.
