Clinical application of three-dimensional and transesophageal echocardiography for occlusion of ventricular septal defect

Abstract:Objective To investigate the value of imaging of three-dimensional and transesophageal echocardiography in indication selection and transcatheter closure of ventricular septal defect (VSD).Methods The type,size and shape of VSD were detected before operation echocardiographilly,occlusions of VSD were monitored during operation and therapeutic effect were observed after operation.Results For patients,32 cases of perimembranous ventricular septal,25 cases of perimembranous defects with inlet extension,3 cases of perimembranous defects with muscular outlet,2 cases of muscular out defects,1 case of myocardial infarction with rupture of interventricular septum.33 occluders were implanted,size of VSD were 3-8 (5.2 ± 0.8) mm,the occluder were 2 mm bigger,the biggest one was 10 mm.30 patients were occludered successfully,no remnant shunt 7 days later by echocardiography.1 with micro-remnant shunt in operation,but dissappeared 24 hours later.1 with little remnant shunt (rate 1.2 m/s,less than 2.0 m/s) just after operation,but only micro-remnant shunt remained 6 months later.Conclusion New imaging of three-dimensional and transesophageal echocardiography can be helpful to detect the size and shape of VSD,guide the size choice and the release of amplatzer occluder during operation,and evaluate the therapeutic effect of occlusion.
