Clinical comparison of direct stenting with predilation implantation in the treatment of saphenous vein graft lesions
Abstract:Objective To evaluate the inhospital and 1-year clinical outcome of patients treated on saphenous vein grafts (SVG) lesions by direct stenting or predilated stenting.Methods From March 2006 to March 2010,105 patients underwent percutaneous intervention on SVG lesions without distal protection devices.Inhospital and 1-year clinical outcomes were recorded and analyzed.34 patients with 46 lesions were treated with direct stenting and 71 patients with 89 lesions were treated with pre-dilated stenting.The inhospital and 1-year follow-up events were recorded and reported.Results The procedural success and inhospital complications had no significant differences.Patients in direct stenting group had less CK-MB elevation >4 times the upper normal value[14.7% (5/34) vs 22.5% (16/71),P < 0.05],and less non-ST-segment elevation myocardial infarction [11.8% (4/34) vs 22.5% (16/71),P <0.05].At 1 year,the major adverse cardiac event (MACE) was significantly lower in the direct stenting group [21.2% (7/33) vs 25.7% (18/70),P < 0.05].Multivariate analysis showed that direct stenting was inversely correlated with MACE (OR =0.47,P < 0.01).Conclusion Direct stenting is better than predilated stenting in treating SVG lesions.
