Clinical effect of prosthetic ring in the correction of Ebstein's anomaly

Abstract:Objective To observe the mid-term and long-term clinical effect of prosthetic ring in the correction of Ebstein's anomaly.Methods Seventy operations for Ebstein's anomaly were performed by the modified procedure.Prosthetic ring was used to reinforce the new tricuspid valvar annulus,but except the no-prosthetic ring group 10 patients.The plastic effect of tricuspid valve was observed real-time by trans-esophagus echocardiography during operation.The follow-up results of these patients were studied after operation.Results Nine patients (90.0%)were followed-up out-hospital from 6 months to 10 years with an average(43 ±41)months in the no-prosthetic ring group.45 patients(80.3%)were followed-up out-hospital from 6 months to 7 years,with an average(24 ± 21) months in the prosthetic ring group.90.8% of these patients in the prosthetic ring group recovered and heart function was improved to class Ⅰ or Ⅱ (NYHA).Conclusions The modified procedure can maintain the farthest dimension and geometric configuration of functional right ventricle.Longitudinal plication of the atrialized right ventricle can restore a large ventricular cavity,improve it's pump function and make valvuloplasty perfect,which widens the indication of tricuspid valvuloplasty for Ebstein's anomaly.It can ameliorate the long-term effect of tricuspid valvuloplasty obviously if using prosthetic ring to reinforce the new valvar annulus.
