Diagnosis of plasma cystatin C for contrast induced acute kidney injury in patients with coronary heart disease combined with renal dysfunction

Abstract:Objective To evaluate the significance of plasma Cystatin C (CysC) for early diagnosis of contrast induced acute kidney injury (CIAKI).Methods All 311 patients with established or suspected coronary artery disease were continuously enrolled in our study.The estimated glomerular filtration rate (eGFR) was more than 30 mE/(min · 1.73 m2) and no more than 90 ml/(min · 1.73 m2) according to the formula of modified diet in renal disease (MDRD).Of 311 patients,39 cases was CIAKI (CIAKI group),and the other patients (272 cases) were ranged in non-CIAKI group.The blood was obtained before and after procedure for detecting the plasma CysC using enzyme linked immunosorbent assay.Results The levels of serum Cr and CysC after operation in CIAKI group and non-CIAKI group were higher than those before operation [CIAKI group:(124 ± 28)mmol/L vs (99 ± 25)mmol/L,(554 ± 199) μg/L vs(457 ± 136) μg/L; non-CIAKI group:(98 ± 21) mmol/L vs(94 ± 20) mmol/L,(469 ±250) μg/L vs(400 ± 183) μg/L,P <0.01].After operation,the levels of serum Cr and CysC in CIAKI group were higher than those in non-CIAKI group(P < 0.05).The area under curve of CysC before and after operation was 0.630[95% confidence intervals(CI) 0.532-0.728,P =0.015] and 0.628(95% CI:0.539-0.716,P =0.016).The sensitivity and specificity of CysC before and after operation was 0.571,0.801 and 0.571,0.602.Conclusion Plasma CysC might be an useful biomarker for prediction of CIAKI with a high negative predictive value.
