Use of amino-terminal portion of brain natriuretic peptide to evaluate prognosis of elderly sepsis patients

Abstract:Objective To study evaluation function of prognosis of N-terminal pro brain natriuretic peptide (NT-proBNP) in elderly patients with severe sepsis and septic shock.Methods Ninety-six cases of elderly severe sepsis and septic shock patients were selected,NT-proBNP and cardiac ultrasound were examined.After admission,according to prognosis the patients were divided into death group (31 cases) and the survival group (65 cases).The difference of NTproBNP levels and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score were compared between 2 groups;The patients were divided into severe sepsis (62 cases) and septic shock group (34 cases) according to the severity of the patients,the difference of NT-proBNP.The left ventricular ejection fraction (LVEF) and APACHE Ⅱ score were compared between 2 groups.Results The NT-proBNP levels and APACHE Ⅱ scores of death group were greater than that in survival group [(3763 ± 1030) ng/L vs (2944 ± 1417) ng/L,(24 ± 6) scores vs (19 ± 8) scores,P < 0.01] ; the level of NT-proBNP and APACHE Ⅱ scores of severe sepsis patients were lower than that in septic shock group[(2896 ± 1325) ng/L vs (3778 ± 1426) ng/L,(20 ±6) scores vs (23 ±6) scores],but LVEF was greater than that in septic shock group (P <0.01).Conclusion The NT-proBNP levels and APACHE Ⅱ score has prognostic evaluation function for elderly patients with sepsis,NT-proBNP level is higher,the prognosis is worse.
