Protective effect of edaravone on brain during carotid endarterectomy

Abstract:Objective To study the cerebral protective effect of edaravone during carotid endarterectomy.Methods All 60 patients scheduled for one lateral carotid endarterectomy were randomly assigned to observation group and control group (30 cases each group).The observation group received edaravone 0.5 mg/kg dissolved in 100 ml 0.9% sodium chloride solution intravenously.The 100 ml 0.9% sodium chloride solution were injected over 30 min after anesthesia induction in control group.The mini-mental state examination (MMSE) value was recorded and compared between before operation and the 3rd day after operation.The anesthesia recovery time,the length of surgery intensive care unit (SICU) stay,and the levels of serum S-100β protein and neuron-specific enolase (NSE)drawn at the time of 10 min after anesthesia induction(T1),and the time of 10 min(T2),2 h(T3),24 h(T4),72 h(T5) after declamping of carotid artery between observation group and control group were compared.Results The levels of serum S-100β and NSE at the time points from T2 to T4 were higher than those of T1 in observation group and control group [S-100β of observation group:(1.03 ± 0.32),(1.70 ± 0.69),(0.69 ± 0.23) μg/L vs (0.35 ±0.06) μg/L,NSE:(11.0 ±2.3),(12.8 ±3.0),(9.6 ± 1.0) μg/L vs (8.2 ± 1.6) μg/L; S-100β of control group:(1.28 ± 0.30),(2.22 ± 0.65),(0.85 ± 0.28) μg/L vs (0.33 ± 0.07) μg/L,NSE:(12.3 ±2.5),(14.8 ±3.0),(10.2 ± 1.2) μg/L vs (8.5 ± 1.4) μg/L,all P<0.05].In observation group,the S-100β and NSE level from T2 to T4 were significantly lower than those of control group (P < 0.05).There was no statistical difference about anesthesia recovery time between the two groups,as well as the length of SICU stay[(83 ±47)min vs (94 ± 74) min,(27 ± 8) h vs (28 ± 9) h,all P > 0.05].The MMSE's value of day 3 after operation was significantly lower than that of basis (preoperative) in both groups [observation group:(25.7 ± 2.3) scores vs (26.3 ±2.5) scores ; control group:(24.7 ± 3.1) scores vs (26.4 ± 2.7) scores,P < 0.05].In observation group,only 3 (10.0%) patients suffered postoperative cognitive dysfunction (POCD),but 9(30.0%) patients suffered in control group (P < 0.05).Conclusions Edaravone has some protective effects on brain,through decreasing the level of serum S-100β and NSE during carotid endarterectomy and after the surgery.It also decsease the cerebral injury,improving the patients MMSE's value,and reducing the incidence of POCD.
