Diagnosis and treatment of esophageal perforation

Abstract:Objective To summarize the diagnosis,therapeutical method and clinical effects of esophageal perforation.Methods From June 1980 to June 2010,54 patients with esophageal perforation were enrolled.Their age ranged from 34 to 69 years with a mean age of 51.5 years.The time of diagnosis was 8-72 hours( 38 patients less than 24 hours,10 patients 24-48 hours,6 patients more than 48 hours).Method included:conservative treatment ( n =2),implantation of covered stent-graft and thoracic drainage ( n =3 ),mediastium or thoracic cavity drainage ( n =20),esophagotomy and excision of foreign body ( n =17),hiatal hernia repair( n =8),esophageal reconstruct and esophagocologastrostomy( n =2),gastrectomy and anastomosis ( n =2).Results 92.6% (46/54) of the patients recovered without any complications,but the death rate was 7.4 % ( 4/54),and anastomotic fistula occured in 2 patients,anastomotic stenosis in 2 patients.Conclusion Early diagnosis is the key in esophageal perforation,surgical therapy is an effective therapeutical method with low morbidity and excellent results.Different therapeutical methods should be chosen according to the time,situs,length and infectious severity of the perforation,complication and general condition.
