Research of pharmacokine of vancomycin in critically ill patients

Abstract:Objective To research pharmacokinetics characteristics of vancomycin of critically ill patients.Methods Totally 8 critically ill patients with normal liver and kidney functions were selected.After 0.5 gram vancomycin was infused for 1 h,the plasma concentrations and urine concentrations were determined by high performance liquid chromatography and analyzed through WINNONLIN software for PK parameters of vancomycin.Results Plasma data showed:the pharmacokinetics characteristics of vancomycin of critically ill patients fited two compartment model; elimination halflife ( 7.08 ± 4.01 ) h,distribution half-life ( 0.49 ± 0.30) h,apparent volume of distribution (0.28 ±0.15)L/kg,maximum concentration (44.46 ±28.60)mg/L,and clearance (63.09 ±31.09)ml/min.Urine data showed:elimination halflife ( 10.47 ± 9.61 ) h,elimination rate constant ( 0.12 ± 0.10 )/h,12 h accumulation of excretion percentage was (46.51 ± 29.17)%.Conclusions The pharmacokinetics of vancomycin in critically ill patients are different from pharmacokinetics of normal population reported before.The critically ill patients should be administrated individually with therapeutic drug monitoring ( TDM ) and the treatment should be adjusted timely to guarantee safety and validity.
