Clinical experience with Perclose ProGlideTM devices during percutaneous endovascular aortic repair

Abstract:Objective To assess the feasibility and safety of percutaneous endovascular aortic repair using a Preclose technique for puncture and closure of femoral access sites.Methods From March 2011 to July 2011,this technique was applied to 151 femoral sites in 120 patients,including descending aortic aneurysm,aortic dissection,penetrating atherosclerotic ulcer and abdominal aortic aneurysm.Preprocedural CTA or clinic syndrome was performed to assess the suitability of common femoral artery and iliaca artery anatomy.Patients were divided into puncture group of 32 cases(42 femoral artery) and surgical control group of 88 cases( 109 femoral artery).Anesthesia,angiography and endovascular aortic repair were performed with standard methods.Low limb braking time,vital signs,femoral puncture sites,ipsilateral lower limb blood flow and CTA imaging of femoral artery after 30 days were evaluated.Results The 32 patients in puncture group(80 males,mean age 67.1 ± 12.3 years) included 6 Stanford B type aortic dissection,4 descending aortic aneurysm,15 abdominal aortic aoeurysm and 7 penetrating atherosclerotic ulcer.Profiles of stent-graft delivery devices used were 16F ( n =20),18F ( n =14),20F ( n =6),22F (n=10) and 24F(n=8).The 88 patients in surgical control group(28 males,mean age 70.2 ±8.9 years) included 22 Stanford B type aortic dissection,40 descending aortic aneurysm,18 abdominal aortic aneurysm,5 penetrating atherosclerotic ulcer and 3 descending aortic aneurysm combined with abdominal aortic aneurysm.Profiles of stent-graft delivery devices used were 16F(n =18),18F(n =19),20F(n =35),22F(n =26),24F(n =18) in the group.Two Perclose ProGlideTM( Abbott USA) devices were used to close 42 femoral puncture sites of puncture group.A femoral artery was failure,and technical success rate was 97.6%.One hundred and mine femoral artery of surgical control group were dissociated by traditional method,and technical success rate was 98.2%.There was no statistically significant difference about technical success rate between two groups ( P > 0.05 ).Lower limb braking time of puncture group was (6.2 ± 1.0)h,and (6.1 ± 1.1 )h in surgical control group.There was no statistically significant difference between two groups(P>0.05).Operative time of puncture group was(1.7 ±0.9)h,and(1.8 ±1.1 )h in surgical control group.There was no statistically significant difference between two groups(P >0.05).All procedures were carried out successful with no related complications.Conclusion This study confirms that perecutaneous endovascular aortic repair using the Preclose technique using Perclose ProGlideTM is safe and effective which can be adopted as an alternative technique of surgically cut-down EVAR.
