The clinical value of domiciliary non-invasive positive pressure ventilation in patients suffering from chronic obstructive pulmonary disease with type Ⅱ respiratory failure during the stable phase

Abstract:Objective To investigate the clinical value of domiciliary non-invasive positive pressure ventilation(DNPPV) in patients suffering from chronic obstructive pulmonary disease(COPD) with type Ⅱ respiratory failure during the stable phase. Methods Thirty patients suffering from chronic obstructive pulmonary disease with type Ⅱ respiratory failure were observed. Pulmonary encephalopathy occurred 1-2 times every year before DNPPV.10 patients were given DNPPV, 2-3 times everyday, every time for 2-4 hours. Ten patients received oxygen therapy.Other 10 patients received routine treatment. Results In 10 patients after DNPPV treatment for 1 year, the arterial pH, PaCO2 and PaO2 were[(7.24 ±0.06) vs (7.34 ±0.07) ;(78 ±9)mm Hg vs (52 ±7)mm Hg;(50 ±7)mm Hg vs (63 ± 5)mm Hg;1 mm Hg = 0. 133 kPa] respectively (all P < 0.01). The mean times of the hospitalization and the mean cost per year in DNPPV group were [(1.98 ± 0.53) times/year, (4.62 ± 1.51) ten thousand yuan/year)]respectively, as compared to the oxygen therapy group [(4.03 ± 0.91) times/year, (12.86 ± 8.26)ten thousand yuan/year)] (P <0.01). But the difference of the mean time of the hospitalization and the mean cost a year in the oxygen therapy group [(4.03 ± 0.91)times/year, (12.86 ± 8.26)ten thousand yuan/year)] was not significant different from the routine treatment group [(4.32 ± 0.68) times/year, (16.71 ± 3.22)ten thousand yuan/year)] (P >0.05). Conclusion DNPPV is a good method to decrease the times of hospital admissions and to improve blood gas tension and life quality in patient suffering from COPD with type Ⅱ respiratory failure.
