Nebulised budesonide for treatment of acute exacerbations of chronic obstructive pulmonary disease

Abstract:Objective To evaluate the efficacy and safety of nebulised budosonide (NB) for acute exacerbations of chronic obstructive pulmonary disease (AECOPD). Methods Eighty-one patients with AECOPD were randomized into three groups (control group 39 cases and intravenous injection group 41 cases and inhalation group 40 cases). Control group received only standard bronchodilator treatment (SBDT) and intravenous injection group received SC (40 mg prednisolone, per day) plus SBDT. Meanwhile inhalation group received NB (2 nag per eight hours) plus SBDT. Spirometic measurements, arterial blood gas analysis and detailed biochemical analysis were carried out at admission, 24 h, 72 h, 7 days and 10 days. Results There was significant improvement of all pulmonary function test and arterial blood gas relevant parameters among three groups after therapy (P<0.05). Arterial blood gas and spirograms recovered faster in intravenous injection group and inhalation group. Improvements of arterial oxygen tension (PaO2) in intravenous injection group and improvements in PaO2 and FEVI% in inhalation group were significant at 24 hours. The first significant improvement in control group appeared in arterial oxygen saturation( SaO2) at 72 hours. All relevant parameters of inhalation group showed no significant difference from intravenous injection group except blood glucose. Conclusion The study demonstrates that nebulised budesonide may be an effective and safe alternative to systemic corficosteroids in the treatment of exacerbations of chronic obstructive pulmonary disease.
