Prognosis of value of ANT for patients with MS receiving PBMV therapy

Abstract:Objective To investigate the changes of plasma atrial natriuretie peptide (ANT) concentration in patients with rheumatic mitral stenosis(MS) before and after percutanous balloon mitral valvuloplasty(PBMV), and its value in prognosis prediction. Methods Thirty-two patients with rheumatic MS undergoing first time successful PBMV were enrolled in this study. The levels of ANP were determined by radioimmunoaasay, mitral valve areas (MVA) were measured by echocardiography before and at 1 day and 18 months after the procedure. Results Before PBMV, the levels of ANP were significantly lower in MVA 0.8 cm2 group than those in group with MVA 0.8 cm2(186.41±76.82 ng/L vs. 247.61±55.05 ng/L, P <0.05 ). Compared with those with cardiac function NYHA class Ⅲ, patients with class Ⅱ had lower ANP concentrations ( 188.63 ± 65.34 ng/L vs. 249.22 ± 67. 18 ng/L, P < 0.05). ANP levels was correlated negatively with MVA and correlated positively with cardiac function before procedure. Restenosis was demonstrated in 3 patients (9.4%)with worse cardiac function, smaller MVA and higher ANP concentration. Conclusion The ANP level is valuable to prognosis.prediction in patients with rheumatic mitral stenosis before PBMV.
