Pro-atrial natriuretic peptide predicts severity and outcome in community acquired pneumonia

Abstract:Objective To observe the value of pro-ANP levels for severity assessment and outcome predic-tion in community acquired pneumonia(CAP). Methods Data of 214 patients admitted to the emergency depart-ment with CAP were analyzed in a prospective study, pro-atrial natriuretic peptide, Procalcitonin, C-reactive protein levels, lnterleukin-6, leukocyte count, clinical variables and the pneumonia severity index (PSI) were measured.Pro-ANP levels were measured with a new sandwich immunoassay. Results Pro-ANP levels, C-reactive protein levels, Interleukin-6 and leukocytecount increased with increasing severity of CAP, based on the PSI score (P>0.01 ). In patients who died during follow-up, pro-ANP levels on admission were significantly higher compared to levels in the survivors [633.88(278.10)versus 1459.52(647.98)μg/L, P>0.01 ]. In a receiver operating char-acteristic (ROC) analysis for survival, the area under the ROC curve (AUC) for pro-ANP was 0.81 , which was significantly higher than proealcitonin, C-reactive protein, Interleukin-6 and total leukocyte count, similar to the AUC of the PSI (0.79). Conclusion pro-ANP is an useful biomarker for the risk stratification of patients with CAP.
