Experience in the diagnosis and treatment of 12 cases of testis germ cell tumors

Abstract:Objective The aim of this study is to provide the reference for the diagnosis and treatment of testicular germ cell tumors(TGCT).Methods A retrospective study of 12 patients with TGCT diagnosed from June 2021 to June 2024 at the Second Affiliated Hospital of Air Force Military Medical University was conducted with their clinical features,treatment methods and outcomes being discussed in conjunction with relevant literature.Results Among the 12 TGCT patients,there were 3 cases of seminoma and 9 cases of non-seminoma germ cell tumors(including 6 cases of mixed germ cell tumors,2 cases of embryonal carcinoma and a case of teratoma).All patients had unilateral lesions,and 6 cases(50%)had a history of cryp-torchidism.There were 7 cases in stage Ⅰ,3 cases in stage Ⅱ,1 case in stage Ⅲ,and 1 case in stage Ⅳ.The immunohisto-chemical results showed that all 6 cases of mixed germ cell tumors expressed CD117,SALL4,Ki-67 and OCT3/4.The serum tumor marker tests revealed that hCG and AFP were elevated to varying degrees in almost all tumors(11/12,92%),which was related to the histological composition of the tumors.All patients underwent surgery,and 5 cases received adjuvant conventional chemotherapy.The other patients had good outcomes after treatment(1 case of lung metastasis and 1 case lost to follow-up).Conclusion The pathological types of TGCT are diverse and judged by immunohistochemistry results.The type of tumor,stage,size and history of cryptorchidism all have varying degrees of impact on prognosis.Comprehensive and accurate pathologi-cal diagnosis is beneficial for the selection of treatment plans and the improvement of prognosis.
