Plaque Characteristics and Changes in Fractional Flow Reserve After Percutaneous Coronary Intervention in Patients With Coronary Heart Disease Combined With Type 2 Diabetes Mellitus
Abstract:Objective To investigate the changes in plaque characteristics and fractional flow reserve (FFR) after percutaneous coronary intervention (PCI) in patients with coronary heart disease combined with type 2 diabetes mellitus (T2DM). Methods A retrospective study was conducted on 189 elderly patients with coronary heart disease treated at the Department of Cardiovascular Medicine of the Affiliated Hospital of Inner Mongolia Medical University from December 2020 to December 2023. The patients were divided into two groups based on whether they had T2DM: the T2DM group (n=105) and the control group (n=84). Plaque parameters on coronary CT angiography (CTA) and FFR were compared between the two groups, and adverse cardiovascular events were recorded after 6 months of treatment. Results After treatment, the plaque length, plaque burden, calcified plaque components, fibrous plaque components, and lipid plaque components were significantly reduced compared to before treatment, while the FFR value was significantly increased (P<0.05). In the control group, the post-treatment plaque length [(6.53±0.74) mm vs (9.81±0.61) mm, P<0.01], plaque burden (0.41±0.02 vs 0.56±0.07, P<0.01), calcified plaque components [(1.11±0.24)% vs (6.13±0.58)%, P<0.01], and lipid plaque components [(3.31±0.45)% vs (5.45±0.41)%, P<0.01] were significantly lower than those in the T2DM group. The post-treatment FFR value was also significantly higher in the control group (0.87±0.02 vs 0.81±0.03, P<0.01). After 6 months, the incidence of adverse cardiovascular events in the T2DM group was significantly higher than that in the control group (19.05% vs 3.57%, P<0.05). Conclusion Patients with coronary heart disease combined with T2DM show less improvement in plaque characteristics and a smaller increase in FFR after PCI, and are more prone to adverse cardiovascular events.
