Predictive value of three serum indicators for secondary acute kidney injury in elderly patients with acute cerebral infarction

Abstract:Objective To analyze the predictive value of combination of serum fibroblast growth factor-23(FGF-23),heart-type fatty acid-binding protein(HFABP),and serum creatinine(Scr)for the occurrence of secondary acute kidney injury(AKI)in elderly patients with acute cerebral infarction(ACI).Methods A retrospective study was conducted on 90 elderly ACI patients admitted in our hospital from January 2023 to July 2024.According to the occurrence of secondary AKI,they were divided into a secondary AKI group(14 cases)and a non-secondary group(76 cases).The clinical data and serum FGF-23,HFABP and Scr levels were compared between the two groups.ROC curve was plotted to analyze the value of serum FGF-23,HFABP and Scr combined together in predicting secondary AKI in the elderly ACI patients.Results The secondary AKI group exhibited significantly advanced age,larger ratios of transfusion history and using β-blockers,higher glycated hemoglobin A1c(HbA1c)level,and lower estimated glomerular filtration rate than the non-secondary group(P<0.05,P<0.01).Statistical differences were observed in fluid input and output between the two groups(P<0.05).The serum levels of FGF-23,HFABP,and Scr were obviously higher in the secondary group than the non-secondary group(P<0.01).The blood transfusion history,negative balance of input and output volume,HbA1c,FGF23,HFABP,and Scr were risk factors for secondary AKI in elderly ACI patients(P<0.05,P<0.01).ROC curve analysis revealed that the AUC values of serum FGF-23,HFABP and Scr were 0.826,0.795 and 0.840,respectively,which were lower than the value of combined the three indicators(0.872,P<0.05).Conclusion Serum FGF-23,HFABP and Scr levels are obviously higher in the elderly ACI patients with secondary AKI,and their combination shows effectively predictive value for secondary AKI.
