Correlation between sensitivity to thyroid hormones and blood lipids in male elderly with diabetes mellitus

Abstract:Objective To explore the association between sensitivity to thyroid hormones and lipid metabolism indicators in elderly male population with diabetes.Methods A total of 230 elderly male patients with diabetes who were admitted to Chinese PLA General Hospital from January 2008 to December 2013 were enrolled.Based on the tertiles of the ratio(NHHR)of non-high-density lipoprotein cholesterol(non-HDL-C)to high-density lipoprotein cholesterol(HDL-C),they were divided into lower(NHHR≤2.17,76 cases),middle(2.18~3.17,76 cases)and upper(NHHR>3.17,78 cases)tertile groups.Blood lipid indicators and thyroid function indicators,such as free thyroxine(FT4),free triiodothyronine(FT3),and thyroid-stimulating hormone(TSH)were detected.NHHR,FT3/FT4 ratio,thyrotropin thyroxine resistance index(TT4RI),thyroid-stimulating hormone index,thyroid feedback quantile-based index by FT4(TFQIFT4),and thyroid feedback quantile-based index by FT3(TFQIFT3)were calculated.Linear regression model was used to explore the correlation of thyroid hormone sensitivity indices and blood lipid indices.Results Statistical differences were observed among the three groups in terms of hyperuricemia ratio,incidence of chronic kidney disease,TFQIFT3 and levels of non-HDL-C,HDL-C,triglycerides(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)(P<0.05,P<0.01).FT3 level was positively correlated with HDL-C(β=0.162,P<0.05),and TSH was positively correlated with NHHR(β=0.123,P<0.01).After adjusting multiple factors,TFQIFT3 was positively correlated with LDL-C and non-HDL-C(β=0.268,P<0.05;β=0.269,P<0.05).Nonlinear test indicated that after adjusting multiple factors,there was no nonlinear correlation between TFQIFT3 and non-HDL-C as well as NHHR(Pnonlinear>0.05).Conclusion In male elderly patients with diabetes,reduced central sensitivity to FT3 is correlated with increased non-HDL-C level,which may increase the risk of blood lipid disorders and cardiovascular disease.
