Predictive value of dynamic electrocardiogram parameters combined with serum factors for ventricular arrhythmia in ST-segment elevation myocardial infarction after percutaneous coronary intervention

Abstract:Objective To explore the predictive value of dynamic electrocardiogram(ECG)parameters combined with serum fibroblast growth factor 21(FGF21)and nerve growth factor(NGF)for ventricular arrhythmia in ST-segment elevation myocardial infarction(STEMI)patients after percutaneous coronary intervention(PCI).Methods A retrospective trial was conducted on 187 STEMI patients admitted to our hospital between June 2022 and December 2023.According to presence or absence of ventricular arrhythmia within 72 h after PCI,they were divided into arrhythmia group(65 cases)and non-arrhythmia group(122 cases).All patients underwent 24-hour dynamic electrocardiography.The ECG data[standard deviation of normal-to-normal intervals(SDNN),root mean square of successive differences(rMSSD),standard deviation of the average normal-to-normal intervals per 5-minute period(SDANN index),QT interval variability(QTV)]were collected,and serum levels of FGF21 and NGF were detected.The risk factors of ventricular arrhythmia were analyzed with logistic analysis,and predictive value of the above indicators was evaluated with receiver operating characteristic(ROC)curve analysis.Results The arrhythmia group exhibited significantly lower SDNN,rMSSD,SDANN-index and QTV,and higher serum levels of FGF21 and NGF than the non-arrhythmia group(P<0.01).Multivariate logistic regression analysis showed that SDNN,rMSSD,SDANN-index,QTV,FGF21 and NGF were independent risk factors of ventricular arrhythmia after PCI in STEMI patients(P<0.01).ROC curve analysis showed that the AUC value of combined dynamic ECG parameters(SDNN,rMSSD,SDANN-index,QTV)and serum FGF21 and NGF in predicting ventricular arrhythmia was significantly greater than that of single indicator(0.852 vs 0.736,0.752,0.718,0.763,0.692,0.740,P<0.05).Conclusion Combination of dynamic ECG parameters and serum FGF21 and NGF shows high predictive value for ventricular arrhythmia in STEMI patients after PCI,which is beneficial to early identify high-risk patients.
